    By submitting this form, the individual(s) named grant permission for photos or videos in which my face (or my child’s face) is clearly visible and recognizable, taken during TUMC events—including worship services, special services, recognition events, musical activities, fellowship gatherings, covenant activities, and similar church functions—to be included in:

    • Unlisted YouTube sermon videos linked on the TUMC website.
    • The TUMC website blog featuring church activities and events.

    To respect your Privacy, if permission is not received for my (or my child’s) individual image to be used in the above formats, then:
    • My individual image will not be included in posted photos or videos.
    • If I appear in a group photo or video, my visible facial image will be cropped, obscured, or blurred when removal is not otherwise possible.
